Early recognition of illness and injury is essential for protecting the welfare of assistance animals and maintaining safe, effective partnerships. Unlike humans, animals cannot verbally describe pain or discomfort. Instead, changes in health are communicated through alterations in behaviour, posture, movement, appetite, physiological function and task performance. Occupational Therapists are uniquely positioned to recognise these subtle changes because they frequently observe assistance animals during functional activities, community participation and everyday occupations.

Recognising health concerns does not require Occupational Therapists to diagnose veterinary conditions. Rather, practitioners must develop the ability to distinguish normal from abnormal presentation, identify potential indicators of illness or injury, document objective observations and facilitate timely veterinary referral. Early identification of health problems may prevent deterioration, reduce pain and suffering, minimise interruptions to assistance work and support better long-term outcomes for both the animal and handler.

The ability to recognise health concerns also supports ethical decision-making. Continuing to work an assistance animal experiencing pain, illness or injury may compromise animal welfare, reduce task reliability and increase risks to the handler and the public. Occupational Therapists therefore have a professional responsibility to recognise situations where an animal’s health may no longer support safe participation in occupational roles.

What Does a Healthy Assistance Animal Look Like?

Before abnormalities can be recognised, practitioners must understand the characteristics of a healthy animal.

Healthy assistance animals typically demonstrate:

  • bright, alert and responsive behaviour
  • willingness to engage with handlers and the environment
  • normal posture and gait
  • appropriate body condition
  • clear eyes and nostrils
  • healthy skin and coat
  • normal respiration without excessive effort
  • good appetite and hydration
  • normal urination and defecation
  • consistent task performance.

These characteristics should always be interpreted in relation to the individual animal, recognising that normal appearance and behaviour vary according to age, breed, species and health history.

Recognising General Indicators of Illness

Many diseases produce general clinical signs before specific diagnoses become apparent.

Common indicators include:

  • lethargy
  • reduced activity
  • reluctance to exercise or work
  • decreased appetite
  • excessive thirst
  • weight loss or gain
  • vomiting
  • diarrhoea
  • coughing
  • sneezing
  • abnormal breathing
  • fever
  • changes in behaviour
  • poor coat condition
  • changes in urination or defecation.

These signs should never be dismissed as simply “having a bad day,” particularly when they persist or occur in combination.

Recognising Signs of Injury

Injuries may occur during work, exercise, transport or everyday activities.

Signs of injury may include:

  • limping
  • non-weight-bearing lameness
  • swelling
  • bruising
  • bleeding
  • wounds
  • abnormal posture
  • reluctance to move
  • vocalisation during movement
  • sensitivity to touch
  • visible deformity
  • reduced willingness to perform trained tasks.

Any sudden change following trauma should be considered potentially significant until assessed by a veterinarian.

Behavioural Indicators of Pain

Pain often presents through behavioural rather than obvious physical changes.

Indicators may include:

  • reluctance to be handled
  • reduced enthusiasm
  • altered sleeping patterns
  • hiding or withdrawal
  • increased clinginess
  • irritability
  • defensive behaviour
  • decreased play behaviour
  • altered grooming
  • changes in facial expression
  • altered posture
  • reduced task performance.

Because behavioural changes frequently precede obvious physical signs, they should always be investigated rather than attributed to poor motivation or training failure.

Assessing Vital Signs

Occupational Therapists are not expected to perform comprehensive veterinary examinations. However, understanding normal vital signs assists practitioners in recognising abnormalities during emergencies or routine observations.

Important parameters include:

  • body temperature
  • heart rate
  • respiratory rate
  • mucous membrane colour
  • capillary refill time
  • level of consciousness
  • hydration status.

Knowledge of these parameters also supports effective communication with veterinary professionals during emergencies.

Recognising Medical Emergencies

Some clinical signs require immediate veterinary attention.

Emergency indicators include:

  • difficulty breathing
  • collapse
  • seizures
  • unconsciousness
  • uncontrolled bleeding
  • severe trauma
  • inability to stand
  • heat stroke
  • suspected poisoning
  • gastric dilation and volvulus (bloat)
  • snake bite
  • severe allergic reactions
  • persistent vomiting with abdominal distension.

Occupational Therapists should recognise these situations, initiate appropriate first aid within their scope of practice and arrange immediate veterinary care.

Observation, Documentation and Monitoring

Objective observation forms the foundation of clinical decision-making.

Practitioners should record:

  • what was observed
  • when it occurred
  • frequency
  • duration
  • severity
  • potential triggers
  • impact on occupational performance.

Documentation should avoid assumptions or diagnostic language.

For example:

Objective observation

“Dog demonstrated intermittent non-weight-bearing lameness of the left forelimb during retrieval tasks.”

Rather than:

“Dog has a sprained leg.”

Objective documentation supports continuity of care and effective interdisciplinary communication.

When Is Veterinary Referral Required?

Occupational Therapists should recommend veterinary assessment whenever:

  • abnormalities are persistent
  • symptoms are worsening
  • work performance changes without explanation
  • pain is suspected
  • injury has occurred
  • emergency signs are present
  • the cause of behavioural change is unclear.

Referral demonstrates professional competence and protects both animal welfare and handler safety.

Scenario

An Occupational Therapist observes that a hearing assistance dog repeatedly pauses before climbing stairs and occasionally skips a step when descending. The handler reports no obvious injury and assumes the dog is becoming “a little lazy.”

Rather than interpreting the behaviour as reduced motivation, the Occupational Therapist documents the observations objectively and recommends veterinary assessment. Radiographs identify early elbow dysplasia. Following treatment, exercise modification and rehabilitation, the dog’s mobility improves and confidence on stairs returns.
